Heart City Health Center is a non-profit, Federally Qualified Health Center (FQHC) providing affordable, high quality, health and dental care to Elkhart County residents. We offer a sliding fee scale based on income and family size for the Uninsured and also accept Medicaid, Medicare and some private insurances.
